How about this fiasco at a startup that shall remain nameless:

A systems engineer installs new SW at a beta site on Friday, and only a small portion of the design/support team is aware of it. On Monday, someone on the team asks the rest of the team to “release” what was already installed, and also generate some upgrade instructions. An email string with an ever-increasing list of recipients starts and the simple request grows exponentially. What about the rest of the document set? What about other beta sites? Why does the link to the SW say -6 when the SE says he installed -4? Should we send drafts, or wait until the documents have been reviewed? By the end of Wednesday, the original question has been lost and a dozen people are saying “Huh? You did what? We’re doing what?”. I wish I had a dollar for every hour wasted with email strings among people who have had very little communication about a very critical subject. I would be wealthy enough to retire.
